Every patient and their family start with a thorough medical evaluation with a Gracey physician, to establish a baseline for each patient's current/ongoing presenting medical issues and illnesses. A physician follow-up visit will be conducted regularly on a quarterly basis, and as needed.
Trained and supervised by our medical director - and based on the initial medical evaluation visit - Gracey's AI then generates a complete and patient-specific care plan, outlining the specific care and interventions the patient will need to receive to achieve their healthcare goals.
Each patient ,and their family, are then matched with a dedicated care coordinator based on their individual preferences and needs, to carry out the care plan and handle all of the patient's ongoing care needs, in full collaboration with the patient's existing care providers (PCP, Specialists, Home Care Team).
